Position Summary: Odyssey House is seeking a Part-Time Intake Coordinator / Case Manager for our Criminal Justice Program!

Days: Monday, Wednesday, and Friday, 8:00 AM to 4:00 PM
Primary Responsibilities: Job Description:

* Conduct and manage client screenings within an appropriate time frame.
* Collect and verify all collateral contact information.
* Collect and manage all legal information (case #’s, certificates, and letters) and inputting into appropriate systems.
* Coordinate admission of clients with the clinical team.
* Create client files for new admits in multiple systems to ensure accuracy and continuity.
* Provide general program information and professionally answer client questions.
* Assist with coordinating referrals if the client is not appropriate for care.
* Complete QA activities to ensure client files and admission paperwork are completed accurately and consistently.
* Communicate and complete training/meeting notes with coworkers on potential QA issues.
* Maintain admission paperwork to comply with program and agency needs.
* Disperse and record quarterly surveys with clients throughout treatment episodes.
* Guide new admits through admissions and program process.
* Enter client funding and ensure accuracy.
* Help with documentation and data entry as needed.
* Responsible for managing and closing client files upon program completion in multiple systems.
* Assist the program director in managing team training, supplies, and distribution of program materials.
* Assist in facilitating psychoeducational groups as needed.
* Other duties as assigned.
Desired Qualifications: Licensure / Education / Prior Experience Required:

Bachelor's degree in related field, or two years of related employment, or program graduate recommended by staff for employment who is in pursuit of licensure:

* Willingness and ability to pass a criminal background check with the Salt LakeCounty Jail
* First Aid and CPR Certification (available at Odyssey House)
* Must comply with annual TB tests, Flu and other immunizations as required
* Must meet employment eligibility
* Must be able to work variable shifts as needed

Physical Demands of the Job: Ability to lift up to 15 lbs., drive (or adequate alternate transportation), sit for prolonged periods, and perform light to moderate physical activity at times.
