Position Summary: Veteran Housing Specialist will work to ensure that any veteran experiencing homelessness in Salt Lake County is engaged and has access to housing resources. This position will ensure that men, women and families who have served our country and are living at The Road Home, on the streets or in a partnering shelter are supported as they overcome homelessness. The goal of the SSVF program is to provide low-income veterans experiencing homelessness or at-risk of experiencing homelessness with a range of supportive services designed to promote housing stability. The Veteran Housing Specialist will focus on street outreach, community partner outreach, and resource center outreach on a regular and frequent basis.  
Primary Responsibilities: Assessing Eligibility
You Will:
Identify low-income veterans and screen them to determine program eligibility for SSVF assistance 
Complete assessments with our vulnerable veteran population for eligibility using a trauma-informed lens 
Update internal tracking and databases in a timely manner after meeting with guests 
Provide direct outreach focused on helping the veteran exit literal or chronic homelessness directly into permanent housing, following a ‘Housing First’ approach
Responsible for helping the veteran create housing stability plans to include the provision of housing placement services, temporary financial assistance, supportive services, and community referrals as needed
Collaborate with program supervisor in ensuring program fidelity, documentation accuracy, and intra-agency collaborations. 
Daily collaboration with all departments, promoting a healthy team environment.   
Maintain client files and eligibility timely and accurately in line with grant requirements. 
De-escalate, empower and advocate for the diverse populations we work with. 
Engage and build rapport with the target population, other community outreach programs, local social services and public benefit agencies, shelters, faith-based and community-based organizations, and local VA facilities
Community Collaboration
You Will:​
Communicate with external and internal staff concerning needed documents for assessments. 
Attended collaboration meetings. 
Create and build relationships with outside agencies to educate and collaborate on funding options. 
Connect with clients referred from outside agencies to connect them with funding.

You Must:
Demonstrates awareness and desire to understand diverse populations.
Ability to contribute to the Agency's commitment to enhancing awareness and appreciation of diverse ethnic and cultural heritages.
Willingness to understand that housing first, harm reduction, low barrier access to resources is best practice and essential to our mission at The Road Home.
Ability to set and maintain good boundaries, using Trauma-Informed Care as a guideline

Must pass a pre-employment background check and drug screen.
Desired Qualifications: Education and Experience
Lived, work or volunteer experience preferred or a degree in a related field
Experience working or volunteering with diverse populations

Physical and Equipment Requirements
Ability to lift 25 pounds.

Ability to sit, stand and walk for at least an hour at a time.

Must be licensed and able to drive an agency vehicle.
