Organization's Mission: The mission of Neighborhood House is to enrich, empower and educate children and adults through quality, affordable day care and support services.
Position Summary: The full-time position for currently licensed CNA involves working with aging and disabled adults in a fun, familial, recreational environment. The candidate will have a key caregiver role for a small group of bilingual clients and participate in the care planning process with the clients, their families, the Adult Program Manager, and/or the Adult Program Director. The candidate will also lead or assist in daily activities with larger groups of clients, relating to all in a positive manner, regardless of age or functional ability. To support effective function of the direct care team, the candidate should be highly responsible, with a focus on timely/regular attendance for scheduled shifts, and collaboration with others. Preferred candidates also reflect a willingness to assist with client care needs or group activities as needed and without prompting.
Primary Responsibilities: Maintain consistent interaction with aging and disabled adults, encouraging client engagement
in all activities.
• Think and work independently and with direction and communicate with staff members in a fastpaced and sometimes stressful environment.
• Ensure clients receive appropriate diet by reviewing their dietary restrictions, food allergies, and
preferences.
• Record vital signs, including blood pressure, pulse, and temperature if indicated.
• Maintain current CNA training and certification.
• Participate in care planning for assigned group of (5-8) clients by (a) attending family conferences
for assigned clients, and (b) providing meaningful feedback on progress / development of
recreational therapy objectives.
• Track and chart daily & weekly progress (for assigned clients) on (a) blood pressure and (b) SMART
objectives developed for individual clients as part of their person-centered care plan.
• Assist in serving two small snacks and lunch daily.
• Transport clients to/from facility, and on outings when needed.
• On rotating basis, lead calendared activities as individually scheduled, including intergenerational
activities.
• Create and maintain good working relationship with assigned clients.
• Assist clients with ambulation if needed, including on & off vans.
• Assist clients with toileting, personal hygiene, dressing, feeding, and other ADL’s as needed for
both male and female clients.
• Assist with set up and clean-up of materials used for daily activities.
• Maintain compliance with ADS Policies and Procedures, as well as policies in the Neighborhood
House Employee Manual.
• Represent professionalism, and maturity
• Ensure client safety and promote client engagement.
• Other duties as assigned.
Desired Qualifications: • At least 18 years of age and a high school graduate or equivalent
• Current CNA certification
• CPR/ First aide Certified
• Salt Lake County Food Handlers Permit
• Up to date, yearly training in working with aging and disabled adults
