Hourly Wage Minimum: $15/hour

Job Summary: Oversees client progression by assessing, developing, monitoring, and evaluating their progress and providing access to resources. Also oversees client-based social programs and develops training materials and protocols for host homes, mentors, and support groups. Coordinates with state agencies & other service providers to enhance HOH support. Promotes the mission and vision of Holding Out HELP to internal and external individuals.
Primary Responsibilities: • Admits and determines clients’ needs by completing interview, intake form and psychological assessment.
• Establishes an individualized plan for client by coordinating weekly/monthly meetings, providing services and referrals for resources, and follows up for accountability. Tracks client in Podio & Self-sufficiency Matrix.
• Provides crisis support for clients’ and interfaces with state agencies when appropriate.
• Communicates clients’ progress at weekly staff meetings.
• Prepares exit interviews when client achieves self-sufficiency.
• Improves clients’ results by studying, evaluating, re-designing processes, implementing changes, rewriting policies and procedures with oversight, with approval by Executive Director.
• Develops, organizes, and runs social events for clients.
• May need to offer support for educational programs from time to time.
• Oversees and supports volunteers in relation to client involvement.
• Prepares reports by collecting, analyzing, and summarizing data.
• Trains & tracks mentors, host families, and volunteers.
• Teaches life skill classes and may be required to film trainings occasionally for online portal.
• Understands risks associated with this role and does not allow fear to compromise work.
• Cleans center & Emergency apartments along with moving furniture when necessary.
• Cross-trained in other areas and willing to offer support when & where needed.
• Continues education through webinars and workshops to keep current with case management trends (approved by Executive Director in advance).
• Meets budget by monitoring expenses & implementing cost-saving actions.
• Maintains confidentiality with staff & clients.
• Builds strong relations with outside agencies for collaboration of resources.
• Enhances the organization’s reputation by accepting ownership of job and explores opportunities to add value and shares positive experiences with public.
Desired Qualifications: • Bachelor’s degree, preferably in sociology or social work - an Associates is acceptable with additional experience.
• Proficient with Microsoft Office and Excel Programs.
• 1-3 years of interfacing with individuals associated with crisis support and/or case management
• Strong verbal and written communication skills including conflict resolution, people management, adaptability in stressful situations, and is proactive not reactive.
• Non-profit organizational experience desired.
• Program development and strategic planning experience.
• Integrity is a must along with a “can-do” attitude promoting team unity.
• Able to work independently and self-manages to achieve goals.
