Job Summary: The mission of the Family Support Center’s LifeStart Village is to move single parents with children from dependency to self-sufficiency. At LifeStart Village, single parents and their children move from homelessness or near homelessness into safe, stable housing where they develop the necessary skills to create a stable, nurturing environment while working toward the goal of self-sufficiency.

The LifeStart Village Case Manager and Program Lead will join the case management team in providing case management services to families living at LifeStart Village (LSV). While the majority of the workload will be case management, additional duties may also be assigned, such as activity planning and program support.
Primary Responsibilities: Conduct and document needs assessments
Collaborate with residents to create goals and individualized case management plans
Build and maintain collaborative relationships with community partners and continued community outreach
Assist residents in accessing any relevant resources such as: healthcare programs, employment services, education resources, housing programs, etc.
Coach residents on how to independently access needed services
Assist residents to establish and maintain eligibility for any financial assistance needs
Provide crisis intervention for clients
Develop and maintain clear, organized case management documentation
Maintain compliance with any agency contracts, policies, and employment guidelines
Collaborate with development staff to monitor program outcomes
Assist with donation deliveries, volunteers, and LifeStart Village events as needed
Attend community events as needed
Attend all relevant staff and community meetings
Assist with childcare during resident meetings as needed
Work with interns
Other duties as assigned
Desired Qualifications: Bachelor's (Preferred)
Case Management: 1 year (Preferred)
Driver's License (Preferred)
18+ years of age
Department of Human Services background check
CPR and First Aid certification (to take once hired)
TB test (to take once hired)
Case management experience (Required)
Experience working with vulnerable populations, 3+ years (Preferred)
Computer literacy (Required)
Microsoft Office (Required)
Utah State Case Manager Certification (Preferred, can obtain after hire)
Ability to lift 25 lbs. Must be able to climb stairs.
