SUMMARY OF POSITION: Under the direction of the Community Support Manager, the Circles Resource Integration Coach performs case management services and ensures Circle Leaders are connected to community resources in order to continue to progress. The Circles Resource Integration Coach is also responsible implementing strategies from the Integrated Community Action Now (ICAN) initiative by receiving training from a community partner, the Prosperity Center of Excellence, and attending existing community meetings.
Primary Responsibilities: ESSENTIAL JOB FUNCTIONS:

1. Perform on-going case management and referral services to Circles Leaders and families using a holistic, comprehensive approach.
2. Work creatively with Circle Leaders and Allies to develop family goals and strategies based on each families’ particular circumstances, beliefs, and behaviors.
3. Work with Community Support Manager and Circles Coordinator to meet Circles requirements, agency goals, and ROMA guidelines. Work as a team with Circles Coordinator and outreach workers.
4. Develop relationships with Circle Leaders, Allies, and community volunteers to connect Circle Leaders to existing community resources.
5. Collaborate with case managers, allies, and other volunteers to assure families’ progress.
6. Develop partnership agreements and collaborative networks with community resources and services.
7. Collect and maintain relevant and timely data for reporting purposes.
8. Maintain timely case management documentation in database system.
9. Attend agency and department meetings and trainings as required.
10. Attend Resource Integration Coach team meetings and trainings with the Prosperity Center of Excellence.
11. Provide for the confidentiality of all services and communications, both written and verbal per agency, programmatic, and funding requirements.
12. Support the achievement of in-kind goals as they pertain to OWCAP and the Community Support department.
13. Other reasonable duties as assigned.
Desired Qualifications: EDUCATION AND EXPERIENCE:
• Bachelor’s degree preferred in related field (Social Work, Child and Family, Psychology, etc.)
• 3+ year’s work experience in a related field
• Experience working with low-income families and community services preferred
• Bi-lingual preferred
• Additional education may be substituted for experience (paid or unpaid) or a combination of both is applicable to meet requirements to be hired
