Job Summary: The Patient Coordinator assists all clinic patients with qualifying and accessing clinic services.
Primary Responsibilities: - Provides quality and efficient healthcare to our patients.

- Seeks opportunities to maximize resources to benefit our patients and the community.

- Is open to new ideas in health care is delivery.

- Enforces policies about who Maliheh Free Clinic serves, what services Maliheh Free Clinic offers, and the rules and standards by which services are delivered.

- Assists in ensuring compliance with policies, procedures, bylaws, regulatory requirements and best practice guidelines.

- Manages and trains volunteers to assist patients in an effective and timely manner.

- Accepts all New Patient incoming calls, pre-screens potential patients for eligibility, and schedules appointments.

- Triages patient need, according to medical director instructions and criteria, to determine if same day appointment is needed, and acts accordingly.

- Assists all New Patient walk-ins, and pre-screens potential patients for eligibility.

- Communicates with the Medical Director and the Charge Nurse regarding the needs of same day patients, and assures that care is provided.

- Redirects patients with needs that are more critical than we can accommodate to appropriate resources for assistance.

- Schedules these patients for a future appointment to assure follow up.

- Redirects patients who do not qualify for our services to an appropriate community partner for assistance.

- Checks in all new patients on their first visit to the clinic.

- Collects appropriate information from the patient, and assures that the patient receives needed services.

- Maintains medical files by assuring that all information is placed in the electronic chart in an organized manner and ensures that all necessary information is maintained accurately.

- Answers calls and schedules patients as needed to assist the front desk.

- Maintains the cleanliness of the reception area and waiting room area.

- Participates in staff and department meetings, as requested by the Executive Director.

Desired Qualifications: - Ability to exercise sound leadership and judgment.

- Ability to communicate effectively, both verbal and written, in English and Spanish.

- Effective interpersonal skills.

- Ability to multi-task and to prioritize tasks.

- Must be English/Spanish Bi-Lingual.

- High school diploma
Medical Assistant or Medical Administrative Assistant training preferred but not required.
