Job Summary: The Case Manager is responsible for working with a caseload of individuals and/or families as assigned by the Clinical Director. The Case Manager works closely with each client and the UHHR team to address the basic needs of clients; and provide individualized case management and referrals to help clients achieve self-sufficiency. The Case Manager is an active and integral member of the Clinical Team.
Primary Responsibilities: Specific responsibilities include:
1) Job Specific Duties:

Direct Client Services
• Meet one-on-one with clients on a regular basis to develop an individualized case management plan
• Provide clients with referrals to services in the community, ensuring that the referral is completed and client is connected
• Coordinate appointments for clients for services that are related to their therapeutic treatment at UHHR
• Work collaboratively with clients on goals that move the client toward self-sufficiency
• Provide interpretation for UHHR services, and for related services outside the office when other interpreters are not available
• Provide transportation for clients as needed

Indirect Client Services
• Know and understand eligibility requirements and documentation for torture survivor and asylum programs
• Keep updated and detailed client files on each client in caseload and clear and concise case notes
• Record and log contact hours for each client
• Respond accordingly to inquiries from the community on UHHR services

2) Client Care and Safety:
• Maintain stringent client confidentiality both in and outside of the office
• Adhere to UHHR security protocol
• Conduct services in a culturally-appropriate manner
• Adhere to code of conduct

3) Agency Responsibilities:
• Attend and participate in regular staff meetings
• Answer phone calls, greet clients and make referrals within the agency
• Contribute ideas to program planning and assist the program in meetings its goals
• Adhere to agency policy and procedure manual and keep updated on changes
• Attend outreach events and present on agency services as needed
• Promote and represent the agency in a positive, pro-active way
• Understand and follow program protocol and chain of command
• Maintain a culture of respect, honesty and dignity with co-workers and clients
• Other duties as assigned

4) Professional Development:
• Attend trainings relevant to job function, as supported by the budget and supervisor

5) Record-keeping and Reporting:
• Maintain client data in the database
• Keep client files and case notes up to date and in accordance with program policies
• Submit reports and all administrative documentation in a timely manner

Supervisory Responsibilities
This job has no supervisory responsibilities; however, may oversee volunteers and/or interns.
Desired Qualifications: Qualifications:
• Two or more years experience providing direct services to culturally and linguistically diverse clients (including internship and/or practicum)
• Ability to interpret for clients in a culturally sensitive manner
• Active membership or participation in local immigrant communities
• Experience, comfort, and sensitivity working in multi-cultural environment is required
• Competency in providing case management to culturally and linguistically diverse clients
Skills and Attributes
• Ability to work well with an interpreter
• Ability to actively participate and work well as a part of a dynamic multi-cultural team
• Knowledge and experience using computers, Microsoft Office and Internet
• Demonstrated interest in issues of immigrant communities, political torture, severe war trauma, social justice and human rights issues
• Written and spoken fluency in English and fluency in a second language common to UHHR clients are required
• Fluency in Swahili, Kinyarwanda, Arabic preferred
• A four-year college degree preferred or equivalent professional work experience

Physical Requirements:
The employee must be able to sit comfortably to perform work. He/she will also walk, stand, bend and carry light items. The employee will be required to drive an automobile. Special physical demands are not required to perform the responsibilities of this position.
