The Street Outreach Case Manager will assist clients on the street to build rapport and encourage youth to access drop-in and shelter services. The team goes out 2-3 times a week to strategic locations in search of youth in need. They will distribute essential supplies (i.e. hygiene kits, blankets, Narcan), offer case management services, and offer access to community resources such as housing and healthcare. The Street Outreach Case Manager will work within a harm reduction and person centered model. All duties are performed with a multidisciplinary and multi-agency team approach. This is a full-time position.
Primary Responsibilities: ESSENTIAL FUNCTIONS include but are not limited to:

· Advocate for individuals experiencing homelessness and living in the community through one on one contact in a street or emergency shelter setting, empower clients, and collaborate with other agencies to provide resources to clients such as housing, employment, harm reduction services, etc…

· Ensure data is being collected and is accurate, maintain electronic records for reporting purposes, evaluation, and program development

· Complete continued education training on street outreach competencies and best practices for street outreach interventions

· Participate in case management meetings and coordinated entry systems

· Use and understand the company’s hands-off de-escalation behavior management techniques as needed with clients

· Use safe physical management skills when exposed to individuals with behavior problems, substance misuse issues, or complex mental health conditions that may include physical acting out

· Exposure to individuals with various states of cleanliness and hygiene

· Exposure to extreme weather such as heat, rain, or snow

· Other duties as assigned
Desired Qualifications: JOB REQUIREMENTS

· Must be a creative, energetic, problem-solver, with a passion for working with young people

· Maintain a positive working relationship with the interdisciplinary team and adequately communicate the resident and staff needs to the appropriate person(s)

· Must be 21 years of age

· High School Diploma or GED required

· Preferred Bachelor's Degree and/or at least 3-5 years of experience working with youth

· Minimum of 400 hours in human services field

· Intermediate to advanced computer skills required

· Certify on Food Handlers, CPR, and First Aid

· Pass a BCI background check

· No history of child abuse or neglect, sex offenses or violent assault

· Must possess a valid Driver's License, clean driving record and must be able to transport staff and/or clients, as needed

· Ability to perform a range of physical motions (walking, kneeling, hiking, lifting and carrying up to 50 pounds) for long periods of time

· Spanish speaking is a plus

Benefits / Why Work for Us: Compensation

· Starting wage of $12.50-$14.00 per hour

· A health insurance stipend of $200 per month (full-time only/awarded after 90 days)

· 1 week per year of Paid Time Off (PTO) to be accrued at a rate of 1.54 hrs per pay period (full-time only/awarded after 90 days)

This position is full-time, sometimes includes working evenings, weekends, holidays and on-call.
