Job Summary: Under the supervision of the Community Support Program Manager, the Housing Coordinator position will provide comprehensive and holistic housing case management, on-going follow-up on referrals and working with a number of community partners. Must maintain full compliance with awarded funding sources, including but not limited to private, federal, state, and local governments.
Primary Responsibilities: ESSENTIAL JOB FUNCTIONS:
1. Provide comprehensive case management addressing issues related to COVID 19.
a) Determine participant eligibility through the intake process following current eligibility requirements from funding sources.
b) Utilize the application process and assessments to determine rental assistance and mental health needs.
c) Provide referrals to service agencies to obtain support services, i.e., food pantry, housing, etc.
d) Provide direct client services as available through Community Support.
e) Provide follow-up on referrals to community resources; offer additional resources if needed.
2. Document comprehensive case management of services to individuals and families.
a) Enter all participant’s application and service information and notes into database.
b) Maintain files, case management records, documentation and database entries.
3. Complete and submit requisitions to fiscal department in a timely manner.
4. Follow confidentiality guidelines for participants.
5. Prepare and submit required documents and reports in accordance with programmatic needs, state, local, and agency guidelines and in a timely manner.
6. Work towards developing an extensive knowledge of social services and community resources for participants.
7. Connect participants with internal and community resources that assist them in achieving goals set out in their case management plan.
8. Represent agency at community meetings as required.
Desired Qualifications: EDUCATION AND EXPERIENCE:
• Preferred Bachelor’s degree
• 2 or more years prior related experience
