Job Summary: Summary of Job: Coordinates and provides medical/clinical services to the severely mentally ill (SMI) patients on the Forensic Assertive Community Treatment (FACT)team. This includes psychiatric medication management, engagement, assessment, differential diagnosis of psychiatric conditions, recovery service planning, education, and risk assessment and discharge planning. Participates as a team member by providing leadership to the team that ensures a No Force First recovery environment and successful achievement of the goals for the FACT team process with all clients.
Primary Responsibilities: 1. Prescribes, administers and monitors psychiatric medications appropriate to the patient’s diagnosis and according to agency’s medication procedures.
2. With other team nurses provides health care for FACT Team participants including re-evaluation and ongoing treatment.
3. Provides nursing and medical care in compliance with licensing regulations and accreditation standards.
4. Gets to know each participant’s needs and provides appropriate treatment and referral. Documents findings in the medical record.
5. On-call rotation: Respond to crisis calls for assessment and pertinent intervention. Report program activities and development, crises, and any other pertinent information to appropriate chain of command. Be available for 24-7 crisis intervention.
