Job Description: The mission of the Family Support Center's LifeStart Village is to move single parents with children from dependency to self-sufficiency. At LifeStart Village, single parents and their children move from homelessness or near homelessness into safe, stable housing where they develop the necessary skills to create a stable, nurturing environment while working toward the goal of self-sufficiency.
Position Description: The Case Manager at LifeStart Village will utilize needs assessments, and consistent, individual case management to assist our 46 residents in their goals towards self-sufficiency. This includes weekly case management sessions with 23 phase one residents and monthly sessions with phase two residents.

Maximum 40 hours/week, Monday – Friday. Some evenings and weekends will be required on occasion.

• 18+ years of age
• Department of Human Services Background Check
• CPR & First Aid certification (to take once hired)
• TB test (to take once hired)
• Conduct & document needs assessments
• Collaborate on goal setting
• Develop written, individualized case management service plans with specific outcomes
• Build and maintain collaborative relationships with community partners and continued community outreach
• Identify services each resident is receiving from other agencies and collaborate to avoid duplication of services
• Assist residents in accessing any relevant resources such as: healthcare programs, employment services, education resources, housing programs, etc.
• Complete referrals and related activities to assist the resident in obtaining necessary services
• Assist residents to establish and maintain eligibility for any financial assistance needs
• Coach residents on how to independently access needed services
• Assess regularly case management plan and adjust accordingly
• Develop and maintain clear, organized case management documentation
• Maintain compliance with any agency contracts, policies and employment guidelines
• Assist with donation deliveries, volunteers and LifeStart Village events as needed
• Attend community events as needed
• Attend all relevant staff and community meetings

• Case management experience (Required)
• Experience working with vulnerable populations (Preferred)
• Computer Literacy: 3 years (Required)
• Microsoft Office: 3 years (Required)
• Utah State Case Manager Certification (Preferred)
• High School Diploma (Required)
• Bachelor’s Degree in Social Work or related field (Preferred)
• Ability to lift 25 lbs.
• Must be able to climb stairs

Training Provided:
Training will take place with the LifeStart Village Program Director during the first few weeks and concurrently along the way. Will attend four hours of learning the agency culture program within the first eight weeks of being hired.
