Job Description: Certified Case Manager Job Position
Case Managers are social service workers who coordinate care for their clients by working with agencies and institutions to ensure that clients get the care they need. Case managers are critical to the success of the clients at The INN Between.

Build rapport with clients.
Ability to provide psycho-social rehabilitation services.
Determine clients’ needs by conducting interviews and administering VI-SPDAT and SPDAT assessments.
Assist clients with applications for Medicare, Medicaid, SSI, SSDI, housing, employment.Assist clients with securing identification documentation.
Assist clients with treatment plans by coordinating schedules, advocating for needed services and entitlements, obtaining additional resources.
Provide behavioral support, deescalate and intervene in crises.
Provide services for medical frail and terminally ill individuals.
Maintain client records by reviewing case notes and logging events and progress in Electronic Client Records Management System and HMIS.
Participate in weekly interdisciplinary team meetings to evaluate client progress.
Collaborate and communicate with community partners to ensure individuals receive appropriate services.

High School Diploma/GED
Mental Health and Substance Abuse Case Manager License preferred
SPDAT Certified or willing to become
Food Handler’s Permit First Aid/CPR Current and valid driver’s license with clean driving record
Good communication skills
State Background Check will be performed
TB Test and Health Evaluation will be required
