Job Description: Job Summary
The Road Home is seeking a highly motivated individual who is committed, compassionate, willing to be challenged, and who wants to make a difference in the lives of others. We, at The Road Home, believe that the people we serve deserve our best every day. The Family Housing Placement Case Manager has the opportunity to work on a pilot project aimed at creating housing outflow from shelter and motels, seeing families through the housing placement process and on into a safe and warm home. This position will develop a new model for service provided to families experiencing homelessness. The pilot project is funded through June 2018 currently.

Duties and Responsibilities
1. Assist families in navigating the shelter and housing resources available in Salt Lake County.
2. Provide families experiencing homelessness with support and resources, as well as achievable goals aimed at quickly resolving their homeless crisis.
3. Work as a team with the Director of Family Crisis Services, Motel Placement Coordinator, and the rest of the case management and housing teams to ensure quick housing placement for families in need.
4. Maintain accurate and timely case notes on each assigned family and stay up to date on their progress toward housing placement.
5. Work with landlords and motel management and shelter staff to ensure a smooth transition for families from shelter to housing.
6. Complete intake assessments on new families entering shelter.
7. Other duties as assigned.

Education and Experience
1. At least one year of relevant experience in data management role.
2. Bachelor’s degree from accredited college or university.

1. Ability to communicate effectively and handle difficult situations with professionalism, compassion and from a Trauma Informed perspective.
2. Excellent verbal and written communication skills.
3. Critical thinking skills and the ability to creatively problem solve.
4. Active listening skills.
5. High degree of comfort with data entry as well as detailed note taking/documentation skills.
6. Ability to work with diverse clients and be culturally responsive. Ability to approach clients who are from different racial, socio-economic and ethnic communities with respect and openness.
7. Ability to remain on task and achieve goals with minimal direct oversight.
8. Excellent time management skills and the ability to prioritize and multi-task.
9. Familiarity with Trauma Informed Care and Housing First philosophies.
10. Proven experience in collaborating with multiple stakeholders to achieve measurable results.

Physical and Equipment Requirements
Should have the ability to lift 25 pounds.
Must be licensed to drive a vehicle.
