Wasatch Homeless Health Care, Inc. (WHHC) dba Fourth Street Clinic is Salt Lake City’s leading nonprofit health care provider for homeless Utahns operating with a core staff of 50 and a volunteer network of more than 150. Annually, the health center provides services to over 5,000 homeless men, women and children with 30,000 medical, behavioral health (including substance abuse), dental services, case management visits; with health and wellness classes to educate patients on maintaining healthy lifestyles. Fourth Street Clinic also has an onsite pharmacy that filled over 70,000 prescriptions in 2016.

Fourth Street Clinic is hiring a Registered Nurse for the Nurse Care Manager position to act as a medical liaison between Fourth Street Clinic and other homeless service providers. In early summer 2017, Fourth Street Clinic announced an expansion of services from the main clinic site downtown to have individualized care at other homeless resource centers where medical services are limited. In addition to medical care, the Nurse Care Manager will do the following:

Core Responsibilities:
· Serve as the primary liaison for residents in various housing/shelter programs, primary medical home, referral partners and all member of care delivery team
· Coordinate required tests and health care services
· Responsible for continuous review of all medical aspects of clients in caseload; include but not limited to: appropriate utilization of services; ensuring continued skilled need; review of documentation in medical record and maintenance of interdisciplinary communication
· Provide triage and consultation for sites case managers
· Document client’s case management plans and on-going activities
· Develop a clinical pathway plan with the multidisciplinary team
· Seek to ensure the continuity of care by avoiding fragmentation and duplication of services.
· Identify client medical needs, current services and available resources; then connect the client to services and resources to meet established goals
· Identify and address client risk factors and/or obstacles to care
· Communicate the care preferences of clients, serving as their advocate, and verifying that interventions meet the client’s needs and goals of treatment
· Develop a client-focused medical case management plan
· Screen clients and/or populations for healthcare needs
· Participates in performance reports of client’s needs
· Develop and provide patient education to client and support staff (as required)
· Provide chronic disease management
· Promote health and well-being of clients
· Assist in site staff training as it relates to medical questions/needs

· Actively participates on health care and homeless management and housing care teams
· Supports and encourages a collaborative culture that values mutual respect, partnership, learning, trust, and fairness
· Models positive and appropriate professional behavior
· Proactive in behavior and fosters that in others
· Participates in professional workgroups and organizations as needed
· Communicates departmental activities, changes and other related activities to appropriate personnel, committees or others as determined
· Participate in strategic planning and annual operations/implementation planning
· Provide strategic financial input and leadership on decision making issues affecting the organization
· Other duties as assigned

· A graduate of an accredited Registered Nurse program and licensed as an RN in the State of Utah
· Minimum 3-5 years of nursing experience
· ACLS/PALS/BLS certified upon hire or within 90 days of hire date
· Ability to multitask and prioritize care of patients
· RN must work well with the clinic team and team(s) associated with external homeless resource center(s)
· Bilingual in English and Spanish preferred
