Job Description: Agency Description: Roads To Independence is a nonprofit, grassroots, advocacy organization by and for people with disabilities. We are an active agency looking to assist individuals with disabilities live as independently in the community as possible. Core services include but are not limited to Advocacy, Information and Referral, Life Skills Training, Peer Support, Recreation, Assistive Technology and Nursing Home Transition and Diversion. We primarily cover Davis, Morgan, and Weber Counties but serve people in Salt Lake and Box Elder counties on the New Choices waiver as well. The New Choices waiver is a Medicaid program that assists individuals to live independently in the community rather than in a nursing home.
The NH Transition Coordinator assists people with disabilities of all ages and types, transition out of nursing homes or other institutions into the community, to their own homes. The Coordinator will assist in skills training, case services, benefits management, assistive technology, advocacy, housing, attendant services, and coordination of the New Choices Waiver ( for more information).

Essential Duties:

1. Assist consumers that live in institutions in developing a discharge plan/Independent Living Plans (ILP’s)/care plan to transition back into the community.
2. Jointly develop individualized ILP’s in setting goals, establishing timelines, and services to be provided. These services may include but not limited to getting assistive technology, finding community services, attendant services, attendant training, First Aid/CPR training, housing, and benefits management.
3. Identify barriers in the community that prevent consumers from being integrated into their communities.
4. Interact with consumers in a professional manner with respect for their rights and maintain strict confidentiality.
5. Coordinate a team of service providers based on the consumers needs to assure a successful transition.
6. Submit all reports, case notes, and other required paper work in a timely manner.
7. Prepare billings.
8. Attend designated meeting and participate in general center meetings.
9. Advocate on issues promoting the independence of people with disabilities.
10. Provide or coordinate life skills training.
11. Disseminate information to promote community integration. This is presenting programs both in the nursing homes and in the community.
12. Coordinate peer services of paid and volunteer peer mentors comprised of consumers who have successfully transitioned to community living.
13. Organize public-private partnerships to provide services needed for this transition groups.
14. Position requires to the employee to go into the community several times per week to meet with consumers.
15. The employee will schedule presentations in Morgan, Davis and Weber and Salt Lake Counties with Nursing or Assisted Living Centers.
16. Other duties as assigned.


• Minimum qualifications require applicant to be a licensed Social Worker (SSW, CSW, LCSW, BSWA, or CSWI).
• Have a general knowledge of medical terminology, diagnosis, and their impact on independent living skills training.
• Management, leadership and organizational skills with the ability to balance multiple tasks, prioritize work effectively and use judgment and discretion in decision-making.
• Ability to travel in a multi-county service area.
• Computer literate.
• Adequate typing speed
• Reliable transportation
• Excellent writing skills.
• Must have an acceptable Criminal Background Check.
• Experience working with individuals with disabilities a plus.
• Personal experience with disability and /or knowledge of disability issues and the consumer/community based IL philosophy a plus.
• Bilingual a plus.
• Person with a disability a plus.

Working Conditions:
Work schedule requires flexibility and is dependent upon program needs. Work is typically performed in an office environmentally controlled setting. The position requires traveling out of area and working flexible hours, occasionally including weekends and evenings.
