Position Summary: Under the supervision of the Community Support Manager, the Community Support Case manager will assist those in poverty to work towards self-sufficiency by providing direct client services, case management, and connection to resources.
Primary Responsibilities: ESSENTIAL JOB FUNCTIONS:
1. Provide comprehensive case management and referral services to individuals and families.
2. Determine the needs of applicants through motivational interviewing and assist in goal setting based on needs, abilities, and interests.
3. Connect participants with internal and community resources that assist them in achieving goals set out in their case management plan.
4. Maintain current knowledge of community resources and other social services in Weber County.
5. Establish and maintain community partnerships and make referrals as needed.
6. Identify and collaborate with community partners to make connections to employment, education, and other resources for participants.
7. Maintain accurate notes and record services in agency database. Record notes, services, goals, and other relevant information in agency database.
8. Assist in the preparation of reports in accordance with programmatic needs in a timely manner.
9. Maintain current knowledge of program requirements and eligibility criteria.
10. Ensure confidentiality policy and practices are followed.
11. Comply with federal and state rules and regulations.
12. Attend meetings and trainings as required
13. Conduct trainings and presentations as assigned
14. Support the achievement of in-kind goals
15. Other reasonable duties as assigned

KNOWLEDGE, SKILLS, AND ABILITIES:
• Knowledge of and ability to perform case management skills.
• Ability to develop and maintain community partnerships.
• Able to learn and utilize databases.
• Able to adapt written and verbal communication skills in interactions with clients, peers, volunteers, and partners.
• Able to operate pallet jacks and drive a box truck.
• Able to lift and carry moderately heavy loads (over 50 lbs.)
• Must be able to prioritize tasks.
• Must be proficient in use of Microsoft office programs.
• Able to operate standard office equipment.
• Preference will be given to bilingual (English/Spanish) individuals.
Desired Qualifications: PREFERRED EDUCATION AND EXPERIENCE:
• Bachelor’s degree in social work or related field preferred
• Four (4) years working experience in related field
