Job Summary: An Ogden based non-profit organization specializing in supportive services for homeless veterans, is seeking a full-time staff member to provide case management services to program participants.

1. Conducts rapid rehousing and homeless prevention services for veterans and their families.
2. Works with clients to develop a Housing Stability Plan to obtain and/or maintain permanent housing.
3. Provides case management services to family members in the veteran household including referrals to appropriate community resources.
4. Conducts home visits to provide case management services
5. Assist in maintaining proper client charts, including gathering, completing and filing all paperwork and case notes.
6. May involve travel to meet with clients in the community.
7. Must be willing to assist with on-call duties after hours and weekends on a rotation basis with other staff members.
8. Work under the direct supervision of the Program Manager and Executive Director.
9. Other duties as assigned.


Associate or Bachelor degree in public administration, social work, social sciences or related field preferred. Prior case management experience helpful. We will consider students working on a degree and/or those without a degree but with extensive case management experience. Excellent organizational and computer skills desired as well as a good knowledge of community resources and services available. Must have a clean motor vehicle record and a clean background check; Veteran preference.

Salary: $16.00-$18.00 per hour DOE plus benefits
