Job Description:

Assist Health Services staff with care coordination, scheduling, medical interpretation, health advocacy for clients, and transportation. This position will typically spend at least 50% of working time in the office and up to 50% directly with clients and community partners, particularly health services providers. Assist other programs as required with interpretation, including Job Development, Case Management, Youth Services, and Refugee Foster Care.

Responsibilities include:

Work closely with the Health Screening Coordinator to ensure successful completion of PPD skin tests, follow-up blood and lab work, and chest x-rays.
Provide interpretation, transportation, and related assistance for refugee clients with Health Screening appointments, follow-up primary and specialized care appointments, including dental and mental health appointments.
Assist with tracking client immunizations and in transportation and follow up with immunization schedules.
Assist the Mental Health Coordinator with coordinating and scheduling mental health appointments, assessments, and education for all eligible clients.
Assist with coordinating ongoing medical care for clients.
Coordinate with multiple calendars and stakeholders to avoid scheduling conflicts and no-shows at appointments.
Complete all necessary documentation for appointments and services provided including billing forms, case noting, and vehicle reporting
Attend regularly scheduled staff meetings.
Perform other duties as assigned.
Work Environment inside/outside, office, warehouse, noise, temperature, etc.

Work will be performed primarily inside and will occur in the office and other locations as needed.
Work includes transporting clients to and from appointments in the community using agency vehicles and public transportation.
Physical Demands including bending, sitting, lifting and driving

Work requires the ability to sit and stand for long periods of time, to walk moderate distances, and to drive.
Skills, knowledge and abilities:

Swahili, Kinyarwanda, Spanish, Farsi, French, or Tigrinya speaker preferred.
Works well with others and independently in safe and appropriate manner.
Demonstrates both problem solving and problem prevention skills.
Able to be sensitive to the needs of clients, clients' families, visitors, co-workers, volunteers, and other persons with whom the employee may interact.
Consistently performs work assignments in a time-efficient manner.
Proficiency in English oral and written communication.
Must maintain confidentiality regarding clients.
Proficiency in computer skills to maintain case files, complete documentation, scheduling and Outlook and Microsoft Office 365.
Personal qualities of openness and respect for co-workers and clients, compassion, graciousness in helping clients to assess and to face their situations and to accept assistance.
Education, training and or experience:

Bachelor's degree from an accredited institution or equivalent experience.
Familiar with and supportive of the mission of Catholic Community Services of Utah and the vision of the Catholic Church articulated by the diocesan bishop.
Experience with multi-cultural and refugee community
Background Screen/BCI requirement

Must be able to pass BCI/FBI background check.
Licenses, certifications and credentials

Current Utah driver's license, proof of auto insurance , 21 years of age or older, a clean driving record, and ability to meet CCS vehicle safety and auto insurance requirements.
Position status and expected schedule

This position is part-time and non-exempt.
Monday through Friday from 8:30 AM to 5:00 PM.
