Position Summary: Utah Community Action is accepting applications for a full time Adult Education Case Manager.

The Adult Education Case Manager is responsible for assisting clients in holistic case management. Clients may be working towards educational, financial or employment goals. This position will provide comprehensive adult education case management, on-going follow-up on referrals and working with a number of community partners.

This Adult Education Case Manager position earns a competitive wage of $17.79/hour. OTHER JOB DUTIES

Excellent use of English (and preferably Spanish), both spoken and written.
Address the individual needs of the clients and secure appropriate interventions. Ensure clients have access to all available direct-client services.
Provide opportunities for clients to identify their strengths and motivations, and use them. Assess social and financial barriers to determine appropriate interventions.
Able to provide consistent and timely follow-up and track their progress, while maintaining accurate and up-to-date case management notes. This includes monitoring on outcomes.
Maintain up to date knowledge of community resources, agencies and referral procedures. (e.g. health, mental health, nutrition, family relationships, job training, child care, transportation, safety, etc.).
Collaborate with staff to ensure that all service areas are integrated into a comprehensive program.
Help to ensure that Adult Education grant timelines are met.
Represent the agency in a professional manner while collaborating with community agencies.
Reliable transportation a must.
Desired Qualifications: QUALIFICATIONS

One year of experience in social service or financial case management is required.
A degree in social services, education, social work or related degree is required.
COVID-19 Vaccination required.
Proficiency in Microsoft Office.
Bi-lingual in Spanish preferred.
Experience entering information in databases, spreadsheets preferred.
